What you should bring and what to do?

CLASS RULES: Please come to class on time, with the required training equipment.

BAIT BAG/TREAT POUCH Treats, lots of pea sized super small morsels must be of really great value to the dog (boiled chicken breast, hots dogs, cheese are examples)

  • A favorite small toy or chew to distract and keep him busy in between lessons
  • Flat collar, buckle or martingale collar and or harness or head halter
  • No pinch, choke or electronic collars
  • 4 to six foot leash (No flexi-leads please)
  • Clicker (will be provided to you at orientation)
  • Towel or mat for your dog to lie on

A COPY WE CAN KEEP OF YOUR PUPS CURRENT VACCINATION HISTORY BY THE FIRST CLASS.

WE LOVE PARTICIPATION SO FOR THE SAFETY OF EVERYONE, CHILDREN UNDER THE AGE OF EIGHT WILL REQUIRE AN ADDITIONAL ADULT TO BE RESPONSIBLE FOR SUPERVISION.

TO PROTECT THE SAFETY OF EVERYONE IN CLASS, CANINES AND HUMANS ALIKE, PLEASE FOLLOW THE BELOW GUIDELINES:

  • Children must be under parental control at all times.
  • Parents must supervise their children during the entire class.
  • No running or yelling will be permitted.
  • Since not all dogs are comfortable around or friendly toward children, children should be instructed to never approach or pet any other dog in class.
Life is Dog, Inc Refund/Cancellation Policy:

Please consider your schedule carefully as no refunds will be given after the start of class. You may be rescheduled into another class within 6 months. When cancellations occur, due to weather or unforeseen emergencies, an additional class session will be added when possible. If you miss a session, please contact us as soon as possible to schedule a makeup session, in another class, as space is available.

ACKNOWLEDGEMENT OF RISKS AND RESPONSIBILITIES

I, the undersigned, recognize that there is an inherent risk of injury or, illness, in any environment associated with group training class. I also recognize that such risks include, without limitation, injuries or illness from fights, rough play, contagious diseases and/or unwanted pregnancies. Knowing these inherent risks and dangers, I warrant that I, or the owner for who I am assigning as an adult guardian, will abide by all safety rules, vaccination requirements, and instructions. I agree by this contract to assume full responsibility and hold Life is Dog, Inc, its officers, directors, agents, employees, and volunteers, including but not limited to Life is Dog, Inc (collectively, “Releasees”), harmless for said pet(s) injury, death or damage as the result of any incident including my or other’s negligence, except tothe extent that the damage or injury may be due to willful misconduct of Releasees. I further agree to hold Releasees harm-less and indemnify them against all damages, costs, fees and business losses resulting from any claim I make or cause to be made against Releasees for which they are not ultimately held to be legally responsible.

OWNER LIABILITY

I, the undersigned, expressly agree to be held responsible for any injury, expense, costs or damages, including but not limited to destruction of furniture, gates, walls, as well as any damage or injury inflicted upon another human or dog, arising out of my own actions or negligence or the actions of my dog. Life is Dog, Inc. makes every effort to provide a safe environment for the care and training of my dog. However, I understand that I am, and will remain, responsible for the actions of my dog at all times. For the protection of all participants we reserve the right to decline entry to any dog who presents behavioral issues that may be dangerous or disruptive to class. Should we determine the dog is not able to participate in the class you will be referred to a private consultation and given a refund for remaining classes.

Payment/Attendance Policy

For Cancellation within 24 hours of class or no show a $7 fee will be incurred, payment is due upon agreement and purchase.

Refund Policy

There are no refunds for services rendered. Refunds are reviewed on a case by case basis and under certain circumstances (to be determined by Life is Dog, Inc).

Weather Policy

In the event of bad weather, please check for an email notification of cancelled classes before leaving home. If you cannot retrieve your email before class, please text us at 508 319-9077 for confirmation and include your name.

Video/Photography Waiver

I give permission for Life is Dog, Inc. to use photographs and video of myself, family members, and my dog(s) taken during training lessons, classes, and at Life is Dog, Inc. sponsored events. These images may be used for advertising and marketing or educational purposes, both in print form and on the internet for the Life is Dog, Inc website, Facebook page or other social media.

Do not allow dogs to greet one another just yet, we will instruct you on appropriate greetings.

Potty your dog or puppy prior to class. If you need to take a break please do so no worries! Ask us if you need assistance.

Vaccine Requirements Classes

For puppies: 2nd distemper plus one week post vaccine, rabies of age eligible, negative for parasites -minimum of 10 days in home prior to coming.

For puppies over 16 weeks, and all adult dogs: Distemper/Parvo or positive immunity titer results, and Rabies

**Please send or bring a copy we can keep of all current vaccine records to your first class, and proof of any updated vaccines during your enrollment period.

OPTIONAL

Canine Influenza H3N2 and H3N8 , and Leptospirosis recommended but not required at this time. You will be notified if the Canine Influenza vaccines become a requirement in the future.
